چکیده

A 29-year-old male Caucasian with no relevant family history presented with a history of constipation, increasing tiredness and a feeling of intense coldness. His performance at sport, notably rugby and cricket, had fallen off badly largely because of weakness and muscle stiffness. His hair was falling out and his skin had become very dry. The clinical diagnosis was of myxoedema. He had a past history of nocturnal convulsions related to heavy alcohol consumption for which he was taking phenytoin sodium 100mg at night, but alcohol intake was now modest. Initial tests of thyroid function were surprisingly high serum total T4 124nmol/1 (reference range 60-160), serum 'free' T4 168pmol/I (reference range 9-28). He was referred to St Luke's Hospital, Guildford, for further investigation.

Low serum thyroxine due to anti-thyroxine antibody.

In one case of untreated Hashimoto's thyroiditis, the serum thyroxine (T4) value, obtained by radioimmunoassay (RIA) employing resin to separate bound and free T4, was significantly lower than that obtained by competitive protein binding analysis (CPBA). The discrepancy was found to be due to the presence of anti-thyroxine autoantibody in the serum. This phenomenon is considered to be of practi...

High serum thyroxine - binding globulin - an important cause of hyperthyroxinaemia BERNARD

Four patients are described who had a significant elevation of serum thyroxine-binding globulin (TBG). Review of these patients indicated that inappropriate therapy for thyrotoxicosis had been given in three cases. A study of our laboratory records revealed that significant TBG elevation was a relatively common phenomenon which occurred in approximately 0 45% of our hospital population. Patient...

Evaluation of new kits for the assessment in vitro of thyroid function by determination of serum total thyroxine, free TBG capacity, and free thyroxine index using Sephadex G-25 and '25I-labelled triiodothyronine and thyroxine

SYNOPSIS A trial was carried out on 134 patients of new kits (Ames Co) using columns of Sephadex G-25 for the determination of serum total thyroxine (Tetralute test) and for the indirect estimation of serum free thyroxine-binding globulin capacity (Trilute test). Both new methods were quicker and easier than the reference resin methods and of similar precision. The two measurements when combine...

Abnormalities of thyroid function tests in hospital inpatients.

Results of thyroid function tests were analysed in 199 clinically euthyroid inpatients with normal serum thyroid stimulating hormone values. Serum total triiodothyronine was less than 1.25 nmol/l in 61.8% of samples, free triiodothyronine less than 3.9 pmol/l in 57.8%, total thyroxine less than 63 nmol/l in 21.1% and free thyroxine less than 9.5 pmol/l in 17.6%. In contrast, thyroxine binding g...
